Patient Forms

All of our new patient paperwork is available online for you to download, complete, and bring with you to your first appointment.

Office Policies

We understand that you cannot schedule illness and injury in your children. For that reason, we build emergency appointments into our daily schedule. Although we also accept walk-in appointments daily, we stop taking walk-ins one hour before lunch and one hour before closing.


If you have a routine appointment scheduled for your child that you cannot keep, we ask that you call our office to reschedule the appointment.


If you are running 30 minutes or more late, please call to reschedule your appointment.


Children in our office must be supervised at all times, and younger children must be accompanied by an adult while in the restroom.

Financial Policy

Payment is due in full at the time of service. We accept the following forms of payment:


  • Cash
  • Personal checks
  • Money orders
  • Visa
  • MasterCard


Bills can be paid by phone by calling (803) 313-3846. Any amount not covered by your insurance is due within 30 days of the time of service.


Failure to pay your account balance will result in collection procedures and, eventually, discharge from the practice.


We are happy to discuss any questions you may have concerning your bill.


For patients who have a co-pay, co-insurance, unmet deductibles, or any other balances, payment is due before services are rendered at each appointment.


For our “self-pay” patients, we offer a reasonable discount, and an estimate of your bill is discussed before services are rendered. Self-pay patients are not billed for services since the balance must be paid in full at the time of service.


Patients with private insurance should have a full understanding of their benefits, as well as any portions that might be the patient’s responsibility. Patients who have secondary insurance must provide a copy of the card and the information of the insured for us to file claims in a timely fashion.


Patients who do not comply with this policy are liable for any debt incurred.
